In consideration of the services of Revival Health LLC., Wellcoaches Corporation, my Coach, any officers, agents, independent contractors, employees, representatives, executors, volunteers, participants, and all other persons, people or entities in any capacity on their behalf, I hereby agree to release, indemnify, and discharge all parties mentioned above, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estate as follows:

1. I Understand and acknowledge that as part of my wellness coaching program, I may be coached to, or it may be suggested that I participate in exercise activities, e.g., general exercise, aerobic training, strength training, flexibility training, etc., that could be potentially hazardous. I also understand that such activities involve risks of injury and even death, and that I am voluntarily participating in these activities and using equipment and machinery with knowledge of the dangers involved The parties named above have a difficult duty to perform, they seek safety but, they are not infallible. They might be unaware of a participant's fitness or ability. The equipment being used, although inspected might malfunction.

2. Therefore I do hereby further acknowledge that I have either had a physical examination and have been given a physician’s permission to participate, or that I have decided to participate in activity and or use of equipment and machinery without the approval of my physician and do hereby assume all responsibility and risks of injury or death from such participation and activities.

3. I certify that I have adequate insurance to cover any injuries I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage to myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.

4. Should any above mentioned parties be required to incur attorney’s fees and cost to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

5. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.

6. I further understand that my certified wellness coach, as applicable, is an independent contractor and not an agent of Wellcoaches Corporation. By signing this document, I acknowledge that if I am hurt during my participation in this activity, I may be found by a court of law to have waived my right to maintain in a lawsuit against the above named parties on the basis of any claim from which I have released them herein. I have had sufficient time to read this entire document. I have read and understood it, and I agree to be bound by its terms.
